#7 – Activity and Participation

“Bodies of evidence and common sense argue that it is activity and participation that are central to the recovery of individuals with chronic conditions, and not merely treatment outcomes.”

Both are important to the patients’ experience of their illness and recovery, but different aspects of the constructs. “Activities” are “goal-oriented” behaviours, and thus reflect engagement in daily life, while “participation” is more “goal-oriented” and reflects engagement in social roles, for example, doing work.

Activities tend to be the focus of interventions, and there is growing evidence that those interventions that are more focused on achieving activities are more effective than those that are more focussed on “activities”. They are often delivered in the context of an intervention that also focuses on participation.

For example, participation in physical activities is important for patients with chronic conditions, but can be difficult to achieve. The WhyPA trial (1) is an example of an intervention which delivered support with activities, and was found to improve patients’ PA.

In a study that compared the two interventions, the control group’s PA increased, but there was no effect of the activity intervention, nor was there any effect on participation. (2)

As with the psychosocial constructs, it is important to understand how the constructs are measured, and what type of measures are used. Researchers and evaluators often focus on the amount of activities achieved, and thus measure activities by counting the number of times a patient engages in a behaviour, for example, how many times they take the stairs.

This measure can be a good indicator of engagement, but is less effective in measuring “time spent”, which is a more direct measure of participation, and hence more important for some patients.

And finally, it is important to note that some activities will be easier to achieve than others. For example, patients with chronic obstructive pulmonary disease will often be able to do more “activities” than patients with more systemic disease, as their disease is more localised.

To help patients manage their chronic conditions, therapists can provide educational materials about the activities that patients need to be able to do (such as breathing techniques) and the activities that patients need to avoid (for example, exercise when they are breathless).
