Web View with multiple text inputs and image upload does not work

I’m trying to build a login form that’s a web view that has the login info and it is connected to my app by a URL Scheme.
My problem is that I want to have a text input for each username, password input, and a submit button. However, when I click the submit button I get a pop up but only the text inputs are there and no username or password inputs. I’ve looked over all my code for anything that could be wrong and I can’t see anything.
Here is my root view:
